Forms - Adding a Form to a Trip or Event
Useful Information
This page guides you through the steps to create a form for parents to complete online.
If you don't have a license for Trips and Events, please contact sales@tucasi.com
To enable the Forms functionality in SCO, please upgrade to the latest version of SCO which can be found here.
Note that the Forms functionality is only available for trips added after Forms have been enabled. For older trips, the Trip tick box functionality will remain.
This page details the following:
1. Create a Form
First create your trip and save it - see Add a Trip or Event.
Do not select Tick Trip available online yet if you wish to first set up Forms to collect consent or other information.
If you do select this option, once you save the trip, it will be available on SCOPAY and on the mobile app with immediate effect. Parents who have set up Parental Alerts will receive a notification and may make a payment before they complete the form.
Click on the Trips and events tab
Click once on the relevant trip
Click on Change trip
Click the Save and configure form button
This will open a new browser.
Select Yes
The Forms window will open.
You can bookmark this page for quick access in future if you wish.
Click Add form
Form Builder (Trip Details)
Select trip
From the drop down select the relevant trip.
Select cut-off date
Select the date the form needs to be completed by (this is optional).
Select cut-off time
Select the cut off time. Parents won't be able to complete the form after the above date and time selected here (this is optional).
Required for attendance
Tick this box if you require the form to be completed before paying for the trip/event. Parents can't pay until the form is completed.
If the trip/event is optional and has a zero cost, the form will need to be completed before they can secure a place.
For school use only
Tick this box if you require the form for school use only. Parents will not see the form online.
Untitled Form
Click here and add a form name. We recommend including the trip name within the form name.
2. Question Types
You are able to create multiple questions when building the form.
There are 4 different question types you can select:
Text
Use this if you would like a short answer from parents.
For example: Contact phone number. The answer will be entered on 1 line.
Paragraph
Use this if you would like parents to add answers over more than 1 line.
The amount of text the parent can enter is unlimited.
For example: listing medical conditions.
Choice
Use this option if parents needs to make a selection.
The number of options is unlimited, but the parent can only select 1 option.
For example: use this for the parent to select Coach or Own transport.
Confirmation
This is a box that parents are required to tick.
For example: use this for parental consent.
3. Creating Questions
Click Add question
Select the question type (see guidance notes above)
Add a Title for the question; for example Consent
Please Note: Each question must have a different title to be able to report on responses
Add Text - this would be a description; for example; Please tick the box to give consent or Please add emergency contact number
Continue to add questions by repeating the above steps.
Click Save once you have added all your questions.
4. Make the Trip available online
Switch back to Schools Cash Office
On the Trips and events tab, select the trip
Select Change trip
Select Trip available online
For an Optional trip, select the pupils on the right hand side only (Online eligibility)
Enter a Minimum online top-up amount, if required
Select Save and exit.
The form is now available online for parents to complete and make a subsequent payment.
5. How does this look for the parent on SCOPAY?
The parent logs in to their SCOPAY account and they will see Complete form against the trip name.
If the Required for attendance option was selected at step 1 above, the parent will not see the MAKE PAYMENT button for this trip until they have completed the form.
The parent selects Complete form and they complete the questions listed.
Once complete, they click on Submit.
They can use their browser's back button to return to SCOPAY.
The form will show as Submitted and they will see the MAKE PAYMENT button.
Note
If the form is showing on the parents SCOPAY as 'expired' this means the Form cut-off date has passed (see Step 1 above).
You can amend the cut-off date and time within the Form builder to make it available again if necessary.
